The purchaser hereby engages /L.D.N.Y.C.Ent. Disc Jockey service. We agree to perform the engagement hereafter provide with all the terms and conditions herein set forth including those entitled "additional Terms and conditions".

ADDITIONAL TERMS & CONDITIONS
1. The AGREEMENT of L.D.N.Y.C.Ent. Disc Jockey service to perform is subject to proven detention by sickness, accident, riots, epidemics, acts of god, or any legitimate
condition beyond our control. A suitable replacement will be provided.
2. THIS CONTRACT constitutes a complete and binding agreement between L.D.N.Y.C.Ent. Disc Jockey service and the purchaser.
3. The equipment use for your occasion will be the responsibility of the purchaser if any damage is done during the event by a guest. The purchaser will be responsible for cost of the repairs. Intls.X______
4. If for any reason the event is canceled or delayed deposits and funds paid for services is non-refundable.
